Please plan to arrive to your appointment at least 10 minutes early to check-in and if required, update medical records. To maintain short wait times for all patients, we may reschedule your appointment if you are more than 10 minutes late.
If you are unable to make your appointment time, please call ahead to cancel, out of courtesy for other patients and the Health & Wellness Center staff.
All outside lab orders should be faxed to the Health & Wellness Center prior to your appointment to minimize your wait time.
